Occipital Neuralgia Caused by Bone Metastasis of Hepatocellular Carcinoma

Abstract

Occipital neuralgia (ON) is a paroxysmal stabbing pain in the occipital nerve territory and usually idiopathic. Some suggested that injury, inflammation, or compression of occipital nerves and its roots might induce ON. Metastasis has
been rarely reported to cause ON. We report an unusual cause of ON due to a bone metastasis of hepatocellular carcinoma (HCC) to craniovertebral junction. A 49-year-old man with HCC complained ON triggered by neck motion or lifting heavy things without any signs of myelopathy and presented a disrupted C2 vertebral body due to metastasis. This rare case broadens the spectrum of ON and indicates the importance of prudent neurologic and radiologic evaluations in ON.
